This communication reports on the dermoscopic characteristics of four pilonidal cyst disease cases, arising from our dermatology outpatient clinic. Based on clinical and histopathological analyses, four patients who sought care at our dermatology outpatient clinic for a single buttock lesion were diagnosed with pilonidal cyst disease. All young male patients displayed nodular lesions, solitary, firm, and pink, close to the gluteal cleft (Figure 1, a, c, e). Upon dermoscopic evaluation of the first patient's lesion, a red, featureless area was observed centrally, consistent with the presence of an ulcer. The peripheral areas of the homogenous pink background (Figure 1b) exhibited reticular and glomerular vessels, delineated by white lines. In the second patient, a yellow, structureless, central ulcerated area was encircled by multiple dotted vessels arranged linearly along its periphery, situated on a homogeneous pink backdrop (Figure 1, d).
